Keeping your eyes healthy is important.  An annual eye examination can help detect a number of eye health conditions, some of which offer no symptoms before they become something more serious.

To request an appointment at our office, please click on the link below.  A short form will help you select a time and day that's most convenient for you!  A member of our staff will contact you as soon as possible to confirm the first available date!

Please remember that this is only a request for an appointment, not a confirmed date and time. 

Please Note:  It may take up to 72 hours to receive and confirm the appointment time you have requested due to weekends or changes in our normal office hours. All cancellations must be done with a phone call to our office at 457.20/20.
